The Madison County Rural Health Council coordinates the delivery of this evidence based Stanford University Patient Education Research Center program which recognizes the underlying issues that impact all people with chronic conditions. Chronic disease includes conditions such as heart disease, diabetes, arthritis, neuropathy, asthma, high blood pressure, stroke, anxiety, depression and pulmonary conditions, to name a few.   

Leaders cover topics such as problem solving, active living with chronic disease, decision making, pain and fatigue management, better breathing and dealing with difficult emotions.   

The program has been shown to help participants increase quality of life, increase exercise, decrease symptoms such as depression and shortness of breath and utilize emergency healthcare less.   

Classes run for 2 ½ hours per week for six weeks at different locations.
